Developed and tested by Partners in Care Foundation, Healthy Moves for Aging Well is a National Council on Aging-approved model health program. The program begins with an initial home visit by a trained professional who assesses the individual’s level of readiness, reviews recommended exercises, and helps the individual establish goals. A volunteer motivational coach then contacts the client weekly via telephone to monitor progress and provide support and encouragement. Healthy Moves for Aging Well is:
